- Patients living in rural areas: 63% less likely to be readmitted than metropolitan patients
- 35-to-44 age group: 19% more likely to be readmitted compared to 18-to-34 age group
- 45-to-64 age group: 39% more likely to be readmitted compared to18-to-34 age group
- 65-to 74 age group: 40% more likely to be readmitted compared to 18-to-34 age group
- 75-to 84 age group: 56% more likely to be readmitted compared to 18-to-34 age group
- Descriptive data showed that areas with 0 to 4 primary care physicians per 100
