Primary Care ACOs and Payment Reform

From Competition to Collaboration: Insights from the ACO Journey

This workshop is aimed at senior leaders preparing to
participate in collaborative partnerships associated with value-based reimbursement models such as ACOs or bundled payments. We look at a virtual ACO that moved from competition to collaboration in rural
North Dakota, beginning with a Community Health Needs Assessment through development of a Community Health Improvement Plan. This is followed by a panel involving senior leaders who have participated in launching ACOs in their respective environments. The session ends with a facilitated dialogue fielding questions from the audience.
