“Because ACOs are accountable for meeting outcomes and efficiency
measures, beneficiaries should be encouraged to use services from
providers within their organization. Using external health services
undermines the ACO’s effectiveness, particularly if the organization
offers that service. This is not to say patient choice of providers
should be reduced, but under such circumstances, out-of-pocket costs
should apply. The AAFP has called for payments for primary care services
to be made on a per-patient basis, with a prospective global payment and
a global care management fee. The global payment would capture 'core
primary care services', and any services provided by the family
physician that fall outside the core primary care suite would be paid on
a fee-for-service basis, through an appropriate bundle, or via another
