A report evaluating CMS' Accountable Health Communities Model, in which beneficiaries in a Geographic Target Area were eligible for navigation assistance if they had a core health-related social need (HRSN) and self-reported two-plus emergency department visits in the previous year, found that baseline ED visits were 28% higher for beneficiaries with 2 HRSNs compared to those with 1, and 18% higher for those with 3 or more compared to those with 2. Then, during the first year following screening, Assistance Track FFS Medicare beneficiaries in the intervention group had 9% fewer ED visits than their control group counterparts.
