Impediments to addressing the problem of high-need, high-cost
patients cited by ACO leaders in a recent study include: "Funding (65
percent); patient engagement (65 percent); actionable data (40 percent);
physician resistance (36.5 percent); geographic challenges (35 percent);
measuring effectiveness (35 percent); ability to scale (35 percent); IT
resources (33 percent); staff recruitment (32 percent); and
prioritization (30 percent)."

How to Develop End of Life Processes for the ACO's Care
Continuum
Over 30 million US citizens are members of an ACO. While ACOs
have strong incentives to adopt advance care planning (ACP) for
long-term success, one out of five ACOs have few or no
end-of-life processes in place. Although ACP can be difficult to
coordinate across care settings, there are processes that have
successfully met quality metrics and achieved cost savings.
