“Like many others, we have recommended that CMS waive the
homebound/certification requirement to allow more expansive use of home
health services, including support for self-management after a certified
episode and access to short-term services that do not require a full
episode. In our Medicare Population Health strategy (including Pioneer
ACO), we are moving from a referral relationship with our Home Health
agency where primary care and home health operated in parallel, to a
model where home health nurses are members of the inte-grated care team,
our “boots on the ground” bringing a unique perspective and set of care
coordination tools. We hope the payment system will evolve to reward
this work; in the short term, waivers to support it would be a helpful
development."
