Between 2 to 3 percent of patients in the Advocate Health Care system account for 27 percent of the total cost of care. To improve outcomes and better control the costs of its most high-risk patients, Advocate Health Care and Advocate Physician Partners in Illinois are participating in an accountable care organization (ACO).
