  “While the CMS ACO announcement provided good news on quality improvement and some positive news on cost savings, a deeper look into the results show that a minority of ACOs are sharing in savings with CMS. Given seven-figure start-up costs and other expenses related to running an ACO, this is unlikely to serve as an incentive to attract new ACOs or encourage existing ACOs to remain in the program. These uneven financial results are less about inefficient care delivery and more about operational problems within the ACO framework -- its attribution method, problematic data sharing, risk scores that cannot increase, and others. Of greater importance, but not mentioned in the release, is whether or not the patients/beneficiaries of the participating organizations had improved outcomes in health status as a consequence of their care being delivered in the context of an ACO. That kind of an ultimate result is the real measure of an ACO’s success and is not necessarily correlated to savings shared.”
