Customers are appropriately demanding value on day one and not an aspiration for value sometime in the ambiguous future.  ACOs ultimately competing on (risk adjusted) total cost of care and measurable quality with appropriately aligned patient benefit designs are a means to sustainable long term value. Current inherent subsidization of high cost, low value providers by high performing ACOs within open access, broad PPO products will become obsolete due to the unsustainable cost trajectory.
