For any capitated arrangement for Medicare patients there are four critical success factors: (1) the ability to engage patients through a variety of tools including health plan design, wellness programs, etc.; (2) the ability to manage chronic conditions such as diabetes, COPD, CHF, etc.; (3) a highly developed primary care model; and (4) information tools that support patient stratification, closing care gaps and transitions of care.”
