  When I managed a medical group in a group model HMO, free standing ambulatory surgery centers provided a great low cost alternative to hospital based outpatient surgery and promoted great patient and physician satisfaction. Unfortunately, in some areas hospitals have managed to eliminate competition by purchasing them.  In other cases they have employed an out of network strategy which has actually made them more expensive then hospitals.  Successful ACO's can create tremendous leverage by displaying the ability to steer attributed patients to select preferred providers.  Provider organizations that can direct patients to low cost providers offer great opportunities to introduce much needed price competition in the health care marketplace."
