“We are concerned with the increasing overlap of various alternative payment models and feel there is a need to determine a long range plan to harmonize them. These overlaps go beyond just ACOs and BPCI. Soon health systems will also face a mix of different payment models in their markets including Next Gen, CJR and CPCI -- all with different quality metrics, legal waivers, etc., making compliance more difficult. The current precedence rules apply different rules to different types of organizations, creating an uneven playing field before the overlap gets even greater. CMS needs to develop a comprehensive strategy that is fair, ensures that providers know for whom they are accountable, and provides appropriate contribution across participants of multiple models.”

Fifteen of the 20 Pioneer ACOs (75 percent) generated savings during the year, compared with 14 of 23 (61 percent) the prior year."
