components that are beyond their control, it is a great challenge to
specify exactly, and in advance, the spending categories that should be
removed. In general, sophisticated ACOs could handle a much larger set
of spending categories, while newer ones could handle only a much
smaller set. Currently, the Medicare ACO programs do not allow this
flexibility because providers are accountable for all spending in Parts
A & B with varying levels of upside/downside risk and no accountability
