21% of patients
surveyed by the Kaiser Family Foundation has noticed an error in
his/her electronic health record (EHR). The type of error
noticed in the medical records ranged from incorrect medical
history (9%); incorrect personal information (5%); incorrect lab
results or other test results (3%); incorrect
medication/prescription information (3%); and billing
errors/issue (1%).
