“State-run Exchanges will increase over the next five years, as will the
private Exchanges. But neither is sustainable, and the broader option
that will be modeled by the states will be the single-payer system.
Vermont is already implementing it. A state-paid system for healthcare
will ease complexity, contract more efficiently and deliver broader
health outcomes across more people, though, like the Exchanges, the
transition will be tough. Medicaid will need to be phased out and a more
prominent focus on mental/behavioral health will have to achieve parity
with clinical health. That will take time. In the interim, state-run
Exchanges, especially for smaller employers, will build out so that more
people will be insured and, hopefully, the health system will be just
