“I believe the best thing to do is have a fixed-dollar premium support
payment paid for everyone, amount depending on the cost of doing
healthcare business in their area (similar to Medicare PPS). For means
testing, have the payment for each person reported to the IRS for
inclusion in the taxable income of that person. Then the Exchanges would
not have to deal with the IRS on finding out people’s reported incomes.
