"With the reelection of
President Obama and the continuation of the Affordable Care Act (ACA),
the biggest challenge of the medical home movement is to keep the focus
on the need for payment for care coordination. Without reimbursement for
care coordination activities, the medical home movement is stymied.
Prepayment contracts, bundled payments and value based payments are
alternatives to a specific care coordination payment. The regulations
from Medicare and other payers should be explicit in support of the care
coordination and team activities of a medical home."
