Physicians and care teams that transform their practice to a medical home need more time, more human capital, and greater funding to operate their practice around the joint principles of the medical home. The current fee-for-service world does not support primary care practice transformation and PCMH. There is not an ICD 9 or 10 code for ‘population health management’ or ‘care coordination,’ but that is what patients need to be healthier and what physician offices need to serve as successful medical homes. The sooner we can move away from fee-for-service payment and get off the hamster wheel of ‘see more, do more,’ the more successful the medical home movement can be.”

Factoid
  "Half of the members were treated by PCMH practices and half were treated in practices that were not recognized. Diabetic patients treated in PCMH practices experienced lower overall medical claims costs, with costs slashed by 21 percent in the first year. This reduction was driven largely by a decrease in inpatient costs, which fell by 44 percent."
