The PCMH is a complex system of care that requires adequate reimbursement to achieve and sustain change. A serious investment in the PCMH through payment reform does not appear likely in 2016. Elliott Fisher, MD in 2009 warned us that the PCMH would fail without a medical neighborhood. How to align specialists, hospitals, and patients in shared decision making is still a major obstacle. Finally, one of the core principles of the PCMH is to be physician-led -- or at least primary care clinician-led. Unfortunately, independent primary care practices are dwindling and many of those that survive do not have the resources to make the PCMH transformation. Payment reform and
regulatory relief are necessary to turn this tide.”
