“When a patient establishes primary care and has ongoing management of their chronic conditions, there is a better outcome. Patients don't get to the point where it is an emergency and need to go to the emergency room as often."
Greg Stadter, Program Director, the Milwaukee Health Care Partnership (MHCP).

The researchers concluded that the 180-day hospital readmission rate of a group within the Camden Coalition [high utilizers of healthcare services who had been targeted with at-home visits, regular phone calls and escorts to medical appointments] was 62.3 percent between 2014 and 2017. By contrast, a control group with similar risks had a readmission rate of 61.7 percent. Although it was slightly lower, the study concluded there was no statistically significant difference between the outcomes for the two groups.
