“We are seeing some innovative partnerships that are showing the evolution of medical homes to medical neighborhoods. For example, community health centers in rural areas are partnering with local farms to provide produce markets for their patients and the surrounding community. We've seen practices working with local restaurants and retailers to provide nutrition and cooking classes, as well as guided tours of grocery stores to help diabetic patients and their families understand healthy food choices. There are also health care and housing organizations working together to ensure that homeless patients suffering from mental illness or substance abuse are cared for during transitions, along with many, many more. In a two-year
randomized trial, primary care physicians who were transitioned to a
medical home model - with support that included practice redesign, care
managers, and a revised payment plan - saw the overall number of care
episodes resulting in ED (emergency department) visits for their
patients reduced by 3.8 ED visits per year, saving approximately $1,900
in ED costs per physician, per year in comparison with a control group
of physicians.
