  In a two-year randomized trial, primary care physicians who were transitioned to a medical home model - with support that included practice redesign, care managers, and a revised payment plan - saw the overall number of care episodes resulting in ED (emergency department) visits for their patients reduced by 3.8 ED visits per year, saving approximately $1,900 in ED costs per physician, per year in comparison with a control group of physicians.
