PCMH Team Based Care Simulation: WMCHealth PPS Primary Care Physicians Workgroup Feb 2017WMCHealth PPS Primary Care Physicians Workgroup produced a simulation exercise to reflect the unique roles of staff in physician's offices of all sizes and capacity. This simulation was designed to apprise workgroup members of the benefits of team-based care which is the second measure in NCQA's PCMH Level 3 recognition. The simulation specifically focuses on PCMH Standard 2D which drills down to the practice team and how defining specific roles, structured communications and and proper care coordination can make the difference between a well-organized and a chaotic primary care practice. |
