"A first wave of early adopter providers, hospitals and physicians, has begun to connect with patients and caregivers via social networks, especially Facebook. The earliest adopters have been entrepreneurial physicians, academic medical centers, and teaching hospitals, some of whom have actually tweeted during innovative surgical procedures. The opportunity for the patient-centered medical home provider is the acknowledgement that health is where people ‘live, work, play and pray.’ In other words, the medical home is essentially the patient’s home, and not the doctor’s office -- it’s where the patient lives, 24x7. Thus, the mobile phone, remote monitoring, and the Internet become essential platforms for connected health, enabling the patient to care for themselves and their loved ones, informed and supported by the provider’s clinical advice."   Humana created one of the first PCMHs in the country in 2007 with Atlanta-based integrated delivery system WellStar. That arrangement resulted in a 6 percent improvement in diabetic and blood pressure management, as well as a 20 percent improvement in management of "bad" cholesterol levels.
