"Care coordination is clearly the ‘GPS’ on the road to lower cost and
higher quality. My only question is whether it can be outsourced. Before
any docs have a conniption, consider (as I understand it) that Harvard
Pilgrim has a shared services model, where the nurses are centrally
hired and ‘loaned out to the primary care sites. I know for a fact
that’s how it worked at Geisinger, where it was the INSURER that hired
the nurses. And then there is the apostasy of paying a disease, … er,
now called ‘care’ management company. The common ingredient in all
approaches is that the non-physician coordinator is on-site, seamlessly
working with the physician. The economic advantage of outsourcing is
that the cost of the FTE doesn’t fall on the PCP and one coordinator can
