"Based on our
experience working in Missouri and Kansas, there are indications our
states will continue to incorporate medical/health home concepts in
their Medicaid programs, regardless of whether they opt into Medicaid
expansion. All states are seeking ways to increase efficiency and cost
savings in their Medicaid programs because they are significant cost
drivers in state budgets. Strengthening chronic disease management and
increasing patient engagement in their health and health care decisions
will improve health outcomes at the patient and population level, and
reduce cost through better management of chronic conditions.
Medical/health home models offer opportunities to achieve these goals,
and a number of demonstration projects in the public and private sectors
are underway that are attracting the interest of state government."

Conducting strong
evaluations of the patient-centered medical home (PCMH) can lead to
better understanding of whether current models work, how they can be
improved, and how they can be implemented and financed for
sustainability. A survey of 26 medical home pilots in 18 states
concluded that only 40 percent of them had well-developed evaluation
