“One of the things that
affect all of the staff in a medical home and patients is just the
immense amount of information that people are expected to have at their
fingertips. Patients have to have all their information and think about
their medical history and act as a conduit across encounters. Physicians
are talking to the person in front of them and trying to remember facts
specific to that patient. The amount of information seems like it is
exponentially increasing. Nurses deal with it, physicians deal with it,
and if someone misses one little piece, it can have huge consequences.
Finding ways to manage information – that to me might be universal to
healthcare, but it is specific to operating a patient-centered medical
home.” - Samantha L. Solimeo, PhD., MPH, Medical Anthropologist, Veteran Health Affairs; Assistant Professor of Internal Medicine, University of Iowa Carver College of Medicine. |

is a particular concern for the Medicare fee-for-service population;
research indicates that 50% of Medicare beneficiaries receive treatment
at four or more practices each year and from seven or more physicians." |
