Patients have to have all their information and think about their medical history and act as a conduit across encounters. Physicians are talking to the person in front of them and trying to remember facts specific to that patient. The amount of information seems like it is exponentially increasing. Nurses deal with it, physicians deal with it, and if someone misses one little piece, it can have huge consequences. Finding ways to manage information – that to me might be universal to healthcare, but it is specific to operating a patient-centered medical home.”
