"It's possible to have a level 3 PCMH that doesn't save money and
actually drives up costs. That's a really big problem and could threaten
to derail the entire PCMH movement. It's time to refocus our PCMH
efforts on care coordination. It's the only intervention that can
generate savings within a few months of concentrated efforts. The other
items in the PCMH model are important and deserve attention, but
clinical transformation needs to demonstrate immediate value.” Jeffrey
