“Payers want to see that
the investment they are making in a multi-payer medical home initiative
is providing a return. One strategy to get multiple payers on the same
page is to frame the development of a common measure set as an
opportunity to identify the metrics that the participating payers
believe will show value. Positive results may also help make the
business case to non-participating payers or self-insured employers who
may be hesitant to pay into a PCMH initiative for their employees.”

The New England
Journal of Medicine has reported that 30-50% of patients leave their
provider visits without understanding their treatment plan. Hospitalized
patients retain only 10% of their discharge teaching instructions. And
chronically ill patients receive only 56% of clinically recommended
health care.
