Independence Blue Cross Study

According to a recent study that used claims data from about 460,000 patients with chronic illnesses who had one or more Emergency Department visits in any given year, it was found that the transition to a medical home was associated with a statistically significant 5% to 8% reduction in Emergency Department visits overall. In particular, patients with diabetes experienced a 9.5% to 12% reduction in Emergency Department visits.
