  “There is a personal participation which is at the individual patient level, and which we call ‘shared-decision making.’ In this dynamic of the PCMH, the patient and/or family ‘grasp’ -– comprehend and ‘take hold’ -– of the plan of care and treatment plan. It is through ‘shared- decision making’ that patient activation and engagement takes place. The patient and/or family ‘own’ the plan of care and actively participate in it execution. At an organizational or institutional level, patient and/or family representatives should be invited to serve on a ‘Community Council’ – as we do here at SETMA -- which meets at least four times a year to discuss practice issues. The provider commitment to the patient and family should be that if the recommendations of the council do not violate ethical standards, statutory requirements, or moral values, they should be enacted. This further increases the patient and family ‘ownership’ of the practice and should strengthen the PCMH.”
