PCMH - Standard Four

The South Carolina Medical Association's Foundation prepared this webinar to assist primary care physicians with their transition to the patient centered medical home. This seminar is the fifth in a series that explain the NCQA standards for a patient-centered medical home. Today's focus is about Standard Four, which is about care, management and support of patients, especially those with chronic conditions that are hard to manage.
