Keeping Track of Your Medical Home Metrics for Payer Reporting

Keeping Track of Your Medical Home Metrics for Payer Reporting

When reporting metrics to payers, it's important to keep population health goals in mind. Robert Chessin, MD explains how this way of looking at metrics can be generalized for all specialties.
• Report on metrics
• Payers will also look at immunization rates
• Keep track of screening tests
• Documentation is key; document your baseline
• Identify the types of reports you'd like to generate
• Develop a spreadsheet of what you are tracking
• Meet weekly to discuss and adjust the spreadsheet
