“I believe that an ACO's
success depends on PCMH homes. I think it would be hard to define a
‘very good traditional practice’ and so the PCMH standards give us a
common basis of comparison. While a practice could fall short of some of
the elements and still be good and valuable practice, I would like to
see the provision of resources to help these ‘very good practices’ which
are doing their best with limited resources and finances. Thus the ACOs
