primary care physician providing hospital care for their patients has
been changing in recent years. There are increasing numbers of patients,
both adults and children, receiving their hospital care from
hospitalists or specialists who are not part of their primary care
practice or medical home. Ever since the origin of the hospitalist
movement there have been problems with communication between the
hospital provider and the primary care provider, if their is one."
