  “Any clinician or facility that participates with a PCMH in a team approach to care delivery is technically defined as a medical neighbor. Therefore, especially in a fragmented system, retail and worksite clinics could operate as medical neighbors. On a practical level, it seems unlikely to achieve this team approach as their business model does not usually align with the comprehensive, coordinated, whole person orientation of the PCMH. We tried to partner with retail clinics to extend our after-hour coverage. No one was interested in our community. Since they fill a niche created by a lack of investment in the medical home model to improve access, I believe they are competitive to the principles, functioning, and growth of the PCMH concept. Given adequate PMPM reimbursement, we could provide the same if not better enhanced access, including novel connections through telemedicine.”
