Delivering MACRA Care Under the Final Rule- Implementation Considerations and Implications

  “In 2012, we rolled out an oncology medical home model in collaboration with five practices in Michigan, known as the the Michigan Oncology Medical Home Demonstration Project. It changed how we paid for drugs, and added a flat care management fee on payments to the doctors. We also agreed to pay or some other things, such as advance care planning, telemedicine visits and consultations with pharmacists. The clinicians also agreed to develop
treatment pathways for certain cases, such as for breast and colon cancer. Most cancers are amenable to treatment in a medical home model. Some have multiple specialties involved, and those patients who need organ transplants obviously need better care coordination... As a result of this initiative, we were able to cut emergency room visits among this cohort of oncology patients in the medical home by more than two-thirds, and cut inpatient admissions nearly in half. So, there is pretty clearly a place for oncology care in the patient-centered medical home.”
