"The
Primary Care Physicians want both to become a recognized PCMH and to
improve patient care, but many are still stuck on the pay-for-volume
treadmill. The PCMH model takes
time, which is money under this type of reimbursement. PCPs already
shoulder a huge burden for quality care in any organization and need the
financial help to start and sustain the undertaking of a PCMH.
Although the 7th
pillar of the Joint Principles of the PCMH involves payment reform,
improved reimbursement hasn’t happened to the degree or the speed that
allows many PCPs to take the next step."

For the fourth year in a row, lack of payer commitment to reimburse care
coordination was cited by a plurality (46.7%) of respondents as the
single most important issue to overcome in terms of widespread
