“Early evidence from
leading PCMH practices suggest much improvement in communication,
especially when a platform of secure online communication has been
established. A recent article in Health Affairs suggested that practices
using online communication actively have high patient and provider
satisfaction, but a lack of reimbursement prevents widespread use of
such communication. Changes in financing away from fee-for-visit
reimbursement will be necessary to realize more gains from PCMH

year in a row, lack of payer commitment to reimburse care coordination
was the most important issue to overcome
for widespread implementation of the medical home model. This year that
answer increased significantly (by more than twelve percentage points)
to 59.1%." |
