"In one recent analysis, we observed the use of social determinants of
health alone to predict readmission risk as being equally as accurate as
using clinical data alone for predicting this risk. The observation
itself wasn’t incredibly surprising considering the vast medical
literature that supports the idea that many influences for
readmission occur outside of a hospital. The surprise came from the
relative ease with which the socioeconomic data was used and the
opportunity these initial results hold for using this valuable insight

providers offer consumer self-service tools, and 44% provide financial
assistance programs for patients enrolled in high deductible health
plans; however, only 19% of providers offer patient education programs
