  "There are a range of definitions for the term "population health" and an even wider range of programs that can be considered "population health" programs. One major class of programs targets persons within an insured population or a cohort served by an integrated delivery system. These "enrolled population" programs usually focus on a "denominator" of persons with, or at risk, for chronic disease and persons in need of preventive screening. Many, though far from all, of these types of interventions have solid scientific evidence indicating a savings of sponsor costs and/or benefit for the consumer."

  "Statistics revealed during the May 2013 annual meeting of Becker's Hospital Review showed that 71% of hospitals were already experiencing reduced Medicare payments because of readmissions. The lost payments nationwide amounted to $15 billion, and three-quarters of readmissions were preventable."
