Differences in the social determinants of health are often directly linked to worse health outcomes. Disparities in health outcomes, such as life expectancy differences, are in large part due to long-standing systemic and structural racism factors across society that affect the communities in which people live and the care that is available, all of which negatively affects their lifelong health."

Factoid
  According to a Commonwealth Fund survey of older adults:

 * 27% of U.S. older adults who experienced discrimination based on their race or ethnicity felt they did not get the care they needed as a result.
 * 48% of those who experienced such discrimination reported being in fair or poor health, compared to 24% of those who did not experience discrimination.
 * Three-quarters of adults who have felt discriminated against have three or more chronic conditions or need help with daily activities.
