According to an
article published by Healthcare IT News, Legacy Salmon Creek
Hospital in Vancouver, Washington, reduced its all-cause 30-day
readmission rate by 24.9 percent and the overall ED visit rate by 81
percent for identified high utilizers, using the Collective Platform, a
real-time, risk-adjusted event notification and care collaboration tool.

Kaiser Permanente survey: 5 Statistics About Social Barriers to
Health in America
1. 28% of Americans reported
experiencing a barrier to health in the past year due to a social need. 2.
21% prioritized paying for food or rent above seeing a doctor or
obtaining medication.
3.
17% lacked transportation to go to the doctor or pick up medication. 4.
13% were sick and had no one who could help. 5. 9%
couldn’t see a doctor regularly because they lacked stable housing.
