This webinar, which was co-led by NACHC
and AAPCHO, presented new analyses of PRAPARE data that
highlight social determinants' relationship with health
outcomes, based on both population-level analyses as well as
patient-level analyses. The webinar highlighted findings that
demonstrate the socioeconomic differences between the general
population and high-risk population and between patients with
controlled and uncontrolled diabetes.
