"The transition to population health and at-risk models is challenging hospitals’ operating margins because some of these hospitals do not have the infrastructure to manage their finances based on population health metrics, and they are having a difficult time shifting their finance models to account for this way of thinking. Compounding this is the fact that hospitals are also challenged with labor and wage pressures because of the tight market for healthcare workers."
 - Saeed Aminzadeh, Chief Executive Officer, Decision Point Healthcare Solutions.

Factoid
The Diabetes Research, Education, and Action for Minorities, or DREAM trial, studied a group of Bangladeshi-Americans, who have a higher rate of Type 2 diabetes than the general U.S. population. The study participants were randomly assigned to either a control group or a group that participated in several education sessions with community health workers. At the end of the trial period, 36.3% of the intervention group achieved control or stabilization of their hemoglobin A1c levels (defined as under seven percent) compared with 24.6% of the control group. In addition, 55.2% of the intervention group also had decreased A1c levels at the end of the study, compared to 42.5% in the control group.
