"Health care outcomes are driven not only by the quality and capabilities of the health care provider, but also by social determinants of health. Good health care addresses things like access to good nutrition, social connections, transportation and more that can limit the ability for a person to get and stay healthy. For example, in-home health visits to help patients who have difficulty traveling or easily obtained referrals to social and community services can really enable success."

In 2018, Humana screened 50,000 of their members in San Antonio markets on social determinants of health to help their providers make appropriate referrals for different resources to their patients. "The integration of SDoH information helped to increase the number of self-reported 'healthy days' by Medicare Advantage members in Humana communities by 2.7 days." By contrast, "Medicare Advantage beneficiaries in communities who did not participate in the program saw a decrease of 0.6 healthy days for the year."

In this presentation, John Moore briefly walks through the evolution of the PHM market and its increasingly integral tie to VBC and future technology and market trajectories. Highlighted topics include EHR versus best-of-breed solutions, FHIR and open APIs, and the increasing role of IT-enabled services.
