  "People with fewer socioeconomic resources – less education, lower income – have less healthy diets, may be less physically active and have poorer quality sleep, all of which lead to the early development of heart disease risk factors. This overabundance of adverse social and environmental factors are major contributors to the disparities. Approximately 26 percent of African Americans are living in poverty, compared with 15 percent in the overall population. The median family income for African American households is $43,151 compared with $66,632 in the U.S. population."

  Researchers analyzed the electronic health records of 690 patients, age 65 or older, who had been hospitalized for heart failure while participating in a larger study about racial and geographic differences including information on nine social determinants of health. After adjusting for age, the study found 79 people (11%) died within 90 days of hospital discharge. The study also found that patients with one social determinant of health factor, as well as patients with two or more social determinants of health, were about three times as likely to die as those who had no social factors.
