The diabetes rate
among American Indians is 400 percent higher than the general public.
For African Americans, the rate is 150 percent higher. American Indian
and African American babies are dying at twice the rate of white babies.

National Academies: 5 Ways Healthcare Organizations Can Address SODH
1. Awareness: identifying the
social risks and assets of specific patients and populations of
patients. 2.
Adjustment: adjusting clinical care to address social determinants of
health.
3.
Assistance: connect patients with social needs to government and
community resources. 4.
Alignment: assess the social care assets in the community, organize
those assets to promote teamwork across organizations, and invest in
assets to impact health outcomes. 5. Advocacy:
advocate for policies that promote the creation and distribution of
assets or resources to address social determinants of health.
