"Healthcare providers at both a primary care level and across the health delivery system have been using rudimentary population health management (PHM) tools for years to better manage patient care, reduce readmissions and costly ER visits and avoid other chronic disease complications. Even though early tools may not have seemed very advanced, the results from their use have been significant. A chronic disease registry, after all, is a just list of patients with a particular disease or comorbidity factor, with the associated demographics and clinical information. But these types of tools and others more advanced have most definitely proven to impact patient care and reduce costs.
