In healthcare, this data -- commonly referred to as social determinants of health (SDH) -- is far more useful and actionable at the person level. For payers and providers, we use person-level SDH data to (1) assess and predict risk, (2) identify impactable patients (that is, those whose outcomes can be impacted by an organizations' specific clinical and social services) and (3) provide guidance about the non-clinical risk factors that, if addressed, can reduce the person's risk. We anticipate that in 2017 more payers and providers will use person-level SDH for more actionable analytics and a higher return on investment."
