"The term ‘population
health’ has arrived in a big way in the hospital systems in our area.
Most are moving from stratification rules now to more robust regression
predictive analytics. The most dominant focus is on preventing
readmissions by identifying those at higher risk prior to discharge and
making sure those identified are enrolled in transitional care programs.
This work is still very rough yet, as these programs are still often in
early development or pilot stages, but I’m seeing some pockets of
success... In one local system, a nurse-run program took readmissions
rates for patients with congestive heart failure from over 22% to just
over 3% in three months.” Julie A. Meek DNS, President, Meek Consulting

IBM reports that
Virginia health system Carilion Clinic has identified 8,500 patients at
risk for developing heart failure in a pilot project ...the results were
achieved through predictive modeling of data in Carilion Clinic’s
electronic medical record, including “unstructured” data such as
clinicians’ notes and discharge documents that are not often
analyzed....The pilot applied content analytics and predictive modeling
to identify at-risk patients with an 85% accuracy rate. The model
identified an additional 3,500 patients who would have been missed with
traditional methods. Excerpted from: Predictive Modeling News, Volume 7 Number 3, February 2014, Industry News: "Carilion Clinic Spots At-Risk Patients with IBM Solution" |
